Salary: $120,000 - $140,000

*FULLY ON-SITE IN MELVILLE, NY

(No hybrid or remote flexibility)

Supervises a team of 8-10 (Intake Coordinators, Account Managers, Outreach Specialists)

Reports To: Chief Operating Officer

JOB SUMMARY:

The Intake Director is responsible for supervising the daily activities of Intake Coordinators. Provide management and supervision of all intake activities related to referrals and start of care case coverage, i.e, approving and/or disapproving referrals, covering new cases, and readmitted /transfer cases. Utilize management reports, (recruitment, client census reports) he/she will ensure the prompt coverage of new cases and monitor the on-call staff’s need for case intervention after office hours, on weekends and on holidays for NEW CASES. This individual is also responsible for conducting routine meetings with Intake Coordinators and VP PCA Operations to discuss new case assignments, incidents and resolution of patient and worker complaints on NEW CASES. Coordinate with the Billing Director and Intake Staff on matters pertaining to patient admissions, patient readmission and related service authorization issues. Liaises with MLTC/MCO and ALL Payors’ representatives on matters pertaining to patient start up, hospitalization and discharge. Reviews and provides prior approval on problematic time sheets. Responsible for directing Intake Coordinators to perform their duties related to audit compliance.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Routinely review and address with Intake staff all daily “Call Out” lists sent out by Payors to ACHC for case coverage.

• Provide management and supervision of all Intake activities related to case coverage, i.e., covering new, readmitted and transfer cases.

• Provide technical assistance to Intake staff when covering “difficult to serve” cases.

• Ensure that Intake Coordinators input accurate information on the organization’s database system.

• Meet with all Intake staff on a weekly basis to discuss new case assignments, new patient and homecare worker incidents and complaints.

• Ensure that Intake Coordinators follow through on scheduling or rescheduling services for new cases, when necessary.

• Collaborate with the Staffing Coordination Unit to identify the designated Staffing Coordinators who will take over the newly admitted patients and ensure continuum of care.

• Liaises with DSS, MLTC and all other Payor service coordinators on matters pertaining to SOC service authorizations and SOC visits. Intake Director, (PCA Operations)

• Provide ongoing communication with Payor Referral Sources on matters pertaining to contract deliverables on case admissions and negotiations on Single Case Agreements.

• Coordinate with the Director of Clinical and Administrative Support Services on assignment of nurses on SOC cases.

• Closely coordinate with the DPS and Nurses regarding crisis intervention needs of “difficult to serve” cases during admission visits.

• Supervise the Intake Coordinator regarding compliance with new patient admissions, readmissions, service suspensions and delayed start up.

• Establish, implement and monitor a system where service authorization issues are addressed even before patients are admitted to ACHC.

• Conduct quality assurance calls to potential patients who will likely decide to change vendor agency.

• Supervise and coordinate with the Intake Coordinator on matters pertaining to securing new, revised and reauthorized service authorizations.

• When necessary, conduct complaint investigation tasks and prepare appropriate documentation. • Recommend and institute progressive disciplinary measures to field employees, CCs and Intake staff who fail to follow ACHC’s standard operating procedures on new cases.

• Provide routine back up when short staffed.

• Provide assistance to the Director for Patient Services on issues impacting timely start of care.

• Perform other functions deemed necessary by the Chief Operating Officer.

QUALIFICATIONS

Minimum Qualifications and Experience Required

• Bachelor’s degree, with 2 yrs. experience in a social service program in a supervisory capacity.

• Master’s degree in behavioral science, Business Administration, Public Administration, preferred.
