Physical Therapist Rehabilitation

Graduate of an accredited Physical Therapy program. Current Physical Therapist license in the state of practice. Experience in skilled nursing facility or geriatric rehabilitation preferred. Strong clinical, communication, and organizational skills.

Evaluate residents' mobility, balance, strength, gait, and functional limitations. Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability. Provide direct therapy interventions, including therapeutic exercises, gait training, and balance retraining. Monitor residents' progress and adjust care plans as necessary to ensure achievement of therapy goals. Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence. Supervise and guide Physical Therapist Assistants and therapy aides as required.
