Responsibilities
- Conduct initial and ongoing physical therapy evaluations for residents in a skilled nursing setting.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
- Administer evidence-based interventions and document patient progress, updating plans as needed.
- Communicate and collaborate with the interdisciplinary team to coordinate care and support discharge planning.
- Educate residents and families on home exercise programs, safety, and strategies to maintain gains.

Requirements
- Active physical therapy license in the United States with eligibility to practice in Washington State.
- Current CPR/BLS certification.
- 1+ year of physical therapy experience; SNF or long-term care experience preferred.
- Graduate of an accredited physical therapy program.
- Ability to work effectively in a skilled nursing facility environment and participate in a temporary travel assignment.
