LPN Position Summary As a LPN at Decatur Wellness & Rehab, you'll be an essential part of our residents' care. Responsibilities include:
- Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
- Participate in admissions, discharges and transfers of residents as required.
- Assess resident's conditions, prepare, and administer medications per physician orders.
- Initiate, review, and update care plans.
- Report any changes in a resident's condition to the nurse supervisor and keep them informed of factors that interfere with being able to perform work as assigned.
- Perform other assigned duties given by clinical or administrative leadership.
- Maintain confidentiality of protected health information, including verbal, written and electronic communications.
- Current unrestricted LPN License for the state of Tennessee
- Current CPR/BLS Certification
