Advanced Practice Provider (NP/PA) - Ponce Health Center - Full-Time

Job Summary The advanced practice provider (APP) at the Ponce de Leon Center provides culturally competent HIV and general primary care as the primary care provider for continuity patient panel with an emphasis on developing trusting partnerships with patients and ensuring continuity of care under the supervision/guidance of attending physician and in collaboration with patients' care team. The APP performs routine follow up visits, walk in/urgent care visits, and comprehensive new patient/re-enrollment visits including antiretroviral initiation/monitoring in patients with advanced HIV disease with significant psychosocial barriers to care. APPs provide consultation to staff nurses, medical staff, and interdisciplinary colleagues, demonstrating clinical expertise, critical thinking, and problem-solving skills to influence the plan of care for patients/families, optimize clinical outcomes, and affect change in the system. Qualifications Master's Degree in Nursing as Advanced Practice Nurse OR Master's Degree as a Physician Assistant by the National Board of Medical Examiners, or the Academy of Physicians Current Georgia Authorization as an Advanced Practice Provider Registered Nurse or Physician Assistant required Basic Life Support (BLS) required Two or more years as a Nurse Practitioner or Physician Assistant Preferred Core Competencies Patient-Centered Care - Demonstrates a commitment to delivering safe, compassionate, and high-quality care that prioritizes the well-being and satisfaction of patients and their families.
