Hours per Shift: 10 hours (4x10's or 5x8's depending on operational needs)

Experience: Minimum of 2 years of case management experience

License: Active nursing license

Certifications: CPR certification preferred

Must-Have: 2 years of case management experience

6 months of discharge planning case management experience in an acute care hospital, preferably in a large organization or teaching hospital with 500 beds or more

Description: Candidates must have at least 2 years of case management experience. Experience should include 6 months of discharge planning case management in an acute care hospital. Ideal candidates will have worked in a large organization or teaching hospital with a capacity of 500 beds or more. This position requires strong communication and organizational skills. Flexibility in scheduling is essential to meet the operational needs of the department. The role will involve collaborating with healthcare professionals, patients, and their families to ensure seamless care transitions. Candidates should possess the ability to work both independently and as part of a multidisciplinary team.
