Patient Navigator / Care Coordinator

As a Patient Navigator, you'll serve as a trusted guide for patients and families, helping them navigate the healthcare system, access critical resources, and overcome barriers to care. In this highly collaborative role, you'll act as a key liaison between patients, providers, and community partners to ensure seamless, patient-centered care and improved outcomes.
What You'll Do
- Advocate for patients and families, ensuring their needs, goals, and preferences are reflected in care plans.
- Serve as a primary point of contact between patients, providers, and other healthcare stakeholders.
- Coordinate appointments, services, referrals, and follow-up care across multidisciplinary teams.
- Help patients understand their care plans and support adherence to recommended treatments.
- Connect patients with valuable healthcare and community resources, including financial assistance, transportation, and support services.
- Educate patients and caregivers on medical conditions, treatment options, and self-management strategies.
- Empower patients to make informed decisions and actively participate in their healthcare journey.
- Maintain accurate documentation of patient interactions, care plans, and resource referrals.

Qualifications:
- Bachelor's Degree in related field of study required; relevant experience can be considered in lieu of degree.
- 1+ years of patient facing, medical office or other related experience strongly preferred.
- Bilingual Spanish speaking strongly preferred.

Schedule and Work Model
- Full-time, Monday through Friday, approximately 8am-4:30pm.
- Hybrid work model: On-site at Massachusetts General Medical Group (Staniford Street in Boston, MA) 4 days per week; remote/work from home 1 day per week.
- Remote workdays require stable, secure, quiet, compliant workspace with MGB issued equipment.
