Recovery Counselor (CST Cobalt) Apply for the Recovery Counselor (CST Cobalt) role at Trilogy . Schedule: Monday - Friday; 8:00 am - 4:30 pm Location: Rogers Park Salary: $22.89 - 26.32 /hr. Sign-on bonus: $1,500. The Recovery Counselor will assess the client’s personal, medical, emotional, social, and environmental situation to plan for treatment through home, community, and office visits. They will provide individually-based motivational treatment and support to assist clients in their recovery from mental illness. The role includes advocating for clients, linking them to community services as needed, and providing client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach. Recovery Counselors will also provide at least 75% community support services, maintain documentation, and offer crisis/on-call coverage as directed. Responsibilities Assess clients’ situation through home, community, and office visits. Coordinate individual recovery plans with clients, support systems, and other care providers. Complete daily progress notes and all required documentation (IMCANS, LOCUS, tracking forms, etc.) in a timely manner. Recognize opportunities to move clients to appropriate levels of care and provide referrals. Assist clients in identifying signs of de-compensation and assess crisis situations, including stabilization through hospitalization if needed. Provide psychoeducation, medication training, and monitoring in accordance with Trilogy policy. Coordinate with team nurse/pharmacy for medication accuracy and administer meds, documenting on MAR. Assist clients in developing independent living skills (personal hygiene, housekeeping, nutrition, shopping). Help clients apply for entitlements (SSI, SSDI, Medicare, Medicaid, LINK). Explain the representative payee process and provide individualized money management services. Accompany and transport clients to appointments, supporting housing, substance use, budgeting, social support, and medication issues. Advocate for clients and empower them to self-advocacy when appropriate. Communicate effectively within the multi-disciplinary team and participate in daily team meetings. Provide on-call and crisis coverage as assigned. Perform related duties and/or projects as assigned. Qualifications Must meet at least one of the following education/experience requirements: Bachelor’s degree in counseling, guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field. Bachelor’s degree in another field with two years of supervised clinical experience in a mental health setting. Licensed Practical Nurse under the Nurse Practice Act (225 ILCS 65). Certificate of Psychiatric Rehabilitation from a DHS-approved program, a high school diploma/GED, and two years’ experience in a mental health setting. CRSS certification through IAODAPCA. Family Partnership Professional certificate from and in good standing with IAODAPCA. Occupational Therapy Assistant licensed under the Illinois Occupational Therapy Practice Act (225 ILCS 75) with one-year experience in a mental health setting. High school diploma or GED and five years of supervised clinical experience in a mental health setting. Valid Illinois Driver’s License with daily access to a vehicle and $100,000/$300,000 auto liability insurance. Ability to work evenings and weekends as directed. Experience working with people with mental health and/or substance abuse diagnoses preferred. Experience in community-based services preferred.
