POSITION SUMMARY:
Equitas Health Pharmacy is an Equitas Health company providing access, education, consultation and management of general retail and specialty pharmaceuticals for patients of Equitas Health and other Ohio providers, as well as the local community. The Clinical Staff Pharmacist will be tasked with performing both the day to day clinical and community pharmacy staffing responsibilities at Equitas Health Pharmacy. The Clinical Staff Pharmacist will be responsible for direct patient care activities and staffing in the community pharmacy; time will be divided between the clinic and retail pharmacy. This shall include the ongoing development and implementation of the following programs: collaborative medication reconciliation programs, and medication access programs, as well as other programs designated by the Director of Pharmacy Services. The Clinical Staff Pharmacist will also work closely with the Clinical and Specialty Pharmacy Program Manager, medical providers and other medical staff to provide therapeutic recommendations for numerous programs, including but not limited to Transgender Care, PrEP, HIV treatment and Hepatitis treatment. The Clinical Staff Pharmacist will also be tasked with medication dispensing on a weekly basis. HOURLY RATE: $54.50 to $65.25ESSENTIAL JOB FUNCTIONS:
Essential functions of the job include, but are not limited to, traveling, driving or having reliable transportation; written communication; supervising and training staff; utilizing a computer for typing, processing and conducting research; attending meetings. The Clinical Staff Pharmacist utilizesworking knowledge and understanding of pharmaceuticals and pharmacy practice standards to deliver outstanding pharmaceutical care to patients and providers; verifies accuracy of prescriptions processed and/or filled by other pharmacy employees to ensure safety and optimal patient outcomes; and presents to small and large groups.MAJOR AREAS OF RESPONSIBILITY:
Provide superior customer service to patients, providers, community members, clients, other employees and referral sources Develop and/or implement collaborate practice agreements for diagnosis or disease state-specific therapeutic programs Provide therapeutic recommendations involving Transgender Care, PrEP, HIV and chronic disease statement management to medical providers or other medical personnel Provide drug information to patients and other healthcare professionals, including proper medication utilization, appropriate administration, potential adverse events and applicable cautions Identify opportunities for medication therapy optimization, including (but not limited) to the following: Identification of therapeutic or ingredient duplication Identification of drug-drug interactions between different health conditions Modifying dosing frequency Medication consolidation (i.e. use of co-formulated products) Adding or removing medication components in regimens Ensuring that treatment regimens are complete as recommended by national guidelines Coverage determination reviews for medications that are non-formulary or are rejecting due to smart edits designed to stop potential drug-drug interactions or therapy/ingredient duplication. Understand and provide guidance on all necessary patient assistance services to ensure patients’ access to prescribed medications Conduct provider outreach for treatment regimens in question in addition to reminders on clinic goals, such as routine office visits, routing lab testing/screenings and medication reconciliation. Provide reminders or guidance to providers and patients regarding office check-ups, lab testing, screening opportunities that may be required for prior authorization requests, medication therapy management reviews, and drug utilization reviews. Provide consultations to patients to ensure their understanding of therapeutic regimens and specifics of administration, potential side effects, allergicprecautions, etc. Utilize skill and experience to effectively deliver specialized pharmaceutical therapy and services, including adherence programs, specialty disease management, patient management andimmunization services Remain current and up to date on new products, therapies, studies, and guideline updates. Support staff training and development as required. Serve as an information resource center for staff pharmacists, nurse case managers, and other Equitas Health employees for PrEP and Transgender Care medication management. Participate in internal, Federal and State audits and URAC/ACHC accreditation surveys. Collaborate with case managers, behavioral health therapists and any other supporting staff to ensure the access and continual adherence to all patients’ medication regimens Precept pharmacy resident(s) to participate in developing the next generation of pharmacists Work with pharmacy students to build strong long-term relationships and consistent referrals Be adherent with all state and federal pharmacy laws as it pertains to pharmacy practice Collaborate with other Equitas Health departments to help patients optimize their health and wellbeing. Staffing tasks include, but are not limited to, the co-management of pharmacy technicians, input into the management of Equitas Health Medical Center’s 340B program and formulary, checking prescriptions for accuracy and clinical appropriateness, inventory management providing, insight and implantation of services that create a high level of patient satisfaction, and working with the other pharmacists to assure maximal financial success of the Equitas Health Pharmacy within its vision. BS in Pharmacy or PharmD. degree or equivalent FPGEC (NABP) Unrestricted Ohio license to practice pharmacy that is in good standing allowing for the dispensing of medications is required. Knowledge, Skills, Abilities and other Qualifications : PGY1 residency or three years of pharmacy experience with direct patient care required Prefer past experience developing and implementing clinical pharmacy services Prefer specialty pharmacy experience Prefer extensive knowledge of HIV/AIDS, Hepatitis C and LGBT Health Excellent written and verbal communication skills. Possesses or willing to obtain education, accreditation and certification, as necessary, to provide the pharmaceutical care and services delivered by Equitas Health Pharmacy, including Medication Therapy Management, AAHIVP credentialing, consultation in specific disease states and administration of injectable agents and immunizations, as necessary Knowledge of health and social service needs of persons living with or affected by HIV/AIDS and/or other specialty disease states. Must have sensitivity to, interest in, and competence in cultural differences, HIV/AIDS, minority health, sexual practices, chemical dependency and a demonstrated competence in working with persons of color, and gay/lesbian/bisexual/transgender community. Proficiency in all Microsoft Office applications required. Reliable transportation, driver's license and proof of auto insurance required.OTHER INFORMATION:
