Home Health Hospice Case Manager
We are seeking an experienced Home Health Hospice Case Manager to provide compassionate care to patients in the home health setting. The ideal candidate will have a solid background in hospice care and be dedicated to enhancing the quality of life for patients and their families.
Responsibilities
- Manage a caseload of hospice patients, providing holistic and individualized care plans.
- Conduct thorough assessments of patients' needs and preferences.
- Collaborate with a multidisciplinary team to ensure comprehensive care delivery.
- Educate patients and families about the hospice services available to them.
- Facilitate communication among healthcare team members, patients, and families.
- Document patient care activities accurately and timely.
Qualifications
- Current and valid Registered Nurse (RN) license.
- Previous experience in hospice care within a home health environment is required.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Compassionate, patient-centered approach to care.
Work Schedule
This position offers a day shift schedule, working five 8-hour days from 08:00 AM to 04:30 PM.
Additional Information
- Mileage will be reimbursed at the IRS rate.
- This role does not involve on-call duties.
