Licensed Occupational Therapist (OT)

Klarus Home Health is seeking a Licensed Occupational Therapist (OT) to join our growing home health team. This PRN/Part-time role offers one-on-one patient care, flexible scheduling, and the opportunity to work independently while fully supported by a collaborative clinical team. This position will cover the Northern Albuquerque (Bernalillo) region up to Santa Fe, NM.

What You'll Do

Perform OT evaluations and ongoing assessments, including OASIS as required

Develop, implement, and revise individualized plans of care

Provide therapeutic interventions for ADLs, ROM, strength, coordination, and adaptive equipment

Educate patients, families, and caregivers

Maintain timely and accurate documentation

Collaborate with physicians and interdisciplinary teams

Supervise COTAs and aides as applicable

Follow infection control and safety standards

What We're Looking For

Required:

Bachelor's degree in occupational therapy

Current New Mexico OT license

Valid driver's license, reliable transportation, and auto insurance

Current CPR certification

Preferred:

1+ year OT experience

Home health experience

Homecare Homebase proficiency

Strong communication skills

Physical & Work Environment Requirements

Ability to lift, bend, push, pull, and stand/walk for extended periods

Manual dexterity and hand-eye coordination

Ability to work independently in patient homes
