Maxim Healthcare is seeking a compassionate and skilled L icensed Practical Nurse (LPN) to provide one-on-one care for an 19-year-old male patient in his home in East Dubuque, Illinois.

The patient requires care involving a G-tube, trach, and PRN ventilator support. A detailed plan of care is provided, allowing you to focus on delivering exceptional patient-centered care.

Monday-Friday Evenings

• Preferred schedule: 3:00 PM - 9:00 PM
• Flexible scheduling available

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care
• Initiate and follow the established plan of care, requesting revisions as patient needs change
• Provide therapeutic, preventive, and rehabilitative nursing interventions
• Monitor patient condition and report significant changes to the physician or supervisor
• Administer medications and treatments as ordered and document patient response
• Support healthcare teaching, care coordination, goal setting, and discharge planning as appropriate

Qualifications

• Current Illinois Licensed Practical Nurse (LPN) license
• Current Health Certificate (per state regulations or contract requirements)
• Current PPD or Chest X-Ray
• Current BLS certification
• One year of RN experience preferred
