Registered Nurse - Hospice

Job Description

Job Summary:

Registered Nurse to deliver compassionate hospice nursing focused on pain and symptom management, individualized care planning, and supportive communication with patients and families. Provide timely crisis intervention and maintain accurate clinical documentation to support coordinated interdisciplinary care.

Responsibilities:

• Assess patients and administer medications/treatments to promote comfort and relieve suffering.
• Develop, implement, and update individualized care plans in collaboration with physicians, social workers, and the care team.
• Educate family caregivers on disease progression, expected changes, and medication management to support safe at-home care.
• Respond promptly to sudden condition changes by coordinating appropriate interventions and crisis support.
• Document patient assessments, interventions, and outcomes accurately and timely in the medical record.
• Provide compassionate emotional and spiritual support, including grief counseling and post-mortem support as appropriate.

Qualifications:

• Hold an active state nursing license and maintain compliance with all regulatory requirements.
• Possess BLS certification (AHA).
• Have at least 2 years of hospice experience.
• Demonstrate strong clinical judgment in pain management and symptom-focused care.
• Collaborate effectively with interdisciplinary team members to coordinate patient-centered plans.
• Communicate clearly with patients, families, and the care team while maintaining accurate documentation.
