What You'll Do:

• Conduct screenings and evaluations of patients with cognitive or ADL physical functioning disorders
• Develop and implement individualized care plans following regulatory and clinical standards
• Administer therapeutic interventions, including ADL retraining, therapeutic activities, exercises, modalities, and assistive devices
• Monitor patient progress, adjust treatments accordingly, and evaluate outcomes
• Collaborate and communicate effectively with healthcare team members, patients, and families
• Maintain accurate documentation in electronic health records
• Participate in meetings, trainings, and continuing education opportunities
• Support and supervise occupational therapy assistants, rehabilitation aides, and students as needed

What You Need:

• Required: Valid Occupational Therapist license, experience in long-term care or equivalent setting
• Preferred: Experience with cognitive and physical rehabilitation, strong communication skills, ability to work independently and as part of a team
