Responsibilities and duties include but are not limited to: Assesses the full scope of patient needs using the nursing process on a continuous basis Observes signs and symptoms and reports changes in the patient’s condition to the physician and/or other appropriate health care professionals Provides teaching to client and client family members Delegates tasks to other Kore Cares personnel in accordance with the South Dakota Nurse Practice Act Uses knowledge and judgement needed to meet nursing needs of the patient Nursing tasks may include but are not limited to: wound dressing, medication set up, monitor medication compliance, assess vital signs, assess skin condition, assess nutritional status, assess mobility, assess usage of glucometer, toe/nail care, catheter care, assess health, provide teaching/health education, communication with physician and pharmacist, blood draws We are an equal opportunity employer and value diversity at our company. Requirements 2 years of nursing experience, preferred Must have active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of SD Must be able to physically perform the duties of the position Must be able to read, write, understand and speak English This position involves driving to clients' homes/apartments; this is a community based role.
