Role Overview The Patient Growth Specialist (internally known as an Outreach Executive) is a community-facing role dedicated to connecting Medicare-eligible seniors with Oak Street Health. The primary responsibility is to generate territory leads and act as brand ambassadors to drive local patient growth. Outreach Executives work closely with Outreach Managers, Directors, and Community Relationship Managers to execute targeted strategies and expand the patient base. Core Responsibilities Sales & Lead Generation: Generate high-quality leads through community events, localized outreach within the Medicare senior community, cold calling, digital channels, and referrals. Persistently engage prospective patients via email, phone, and face‑to‑face networking; build a robust sales pipeline. Stay current on service updates and marketing initiatives through team briefings. Relationship Management: Document all customer interactions and sales activities within the CRM and internal business systems. Serve as the primary liaison between clinical teams and new patients to ensure a seamless transition. Address and resolve prospect concerns with empathy, maintaining consistent touchpoints to foster trust. Qualifications Minimum 2+ years in sales or healthcare environments. Demonstrated ability to cold call, manage digital lead channels, and influence key stakeholders. Exceptional verbal and written communication skills with a professional phone presence. Strong organization skills, ability to navigate external barriers, and manage multiple priorities in a fast‑paced environment. Valid driver’s license and reliable transportation. Role requires 60% field‑based, 40% clinic‑based activities and flexibility to work weekends for community events and patient engagement. US work authorization required. Preferred: strong computer skills (Microsoft/Google Suite), experience with CRM/EMR software, and fluency in another language is a plus. Compensation & Benefits Pay Range: $18.50–$35.29 per hour or equivalent base annual salary, depending on experience, education, geography, and other factors.
