Patient Care Coordinator Full Time Clerical Iowa City, IA, US Position Summary The Patient Care Coordinator ensures seamless coordination of infusion and enteral services, manages therapy changes, and oversees the resupply of medical equipment, medications, and supplies for patients. This role requires strong communication, multitasking, and problem-solving skills while working closely with patients, caregivers, providers, pharmacies, nursing agencies, and insurance companies. Position Responsibilities Coordinate care for newly admitted patients requiring infusion or enteral therapy. Communicate with pharmacists to determineappropriate drugdelivery methods and resolve medication-related needs. Conduct patient resupply calls, assessing supply requirements and completing clinical screening questions. Ensure compliance withpayerguidelines and regulatory standards for resupply processes. Obtain prior authorizations and verify insurance coverage to supporttimelyservice. Identifynursing care needs and coordinate services withappropriate homehealth or nursing agencies. Monitor and address irregular supply or medication usage patterns, resolving shipping or service issues as needed. Collaborate with referral sources, clinical teams, and inventory departments to ensure efficient andaccurateservice delivery. Participate in quality assurance initiatives, process improvements, and complete required documentation foraccuratebilling. Otherduties as assigned. Education and Experience Requirements Highschooldiploma orGeneral Education Development (GED)equivalent. Experience in healthcare, customer service, orrelatedfield. Desired Qualifications and Experience Knowledgeofhomecare infusion or enteral nutrition services. Experience withprior authorization processes and payer requirements. Experience with medical supply resupply processes and insurance verification. Familiarity withEPIC or Brightree. Required Skills and Abilities Strong attention to detail and ability to multitask in a fast-paced environment. Excellent written, verbal, and interpersonal communication skills. Proficientin MicrosoftOfficeand electronic medical record (EMR) systems. Creative problem-solving and customer service orientation. Demonstrated ability to build cooperative working relationships and contribute to team success. Physical Requirements These physical demands are representative of those that must be met by an employee to successfully perform the essential functions of the job. Ability tolift20 lbs. Ability to sit for extended periods, including an 8-hour shift. Additional Eligibility Requirements Successful completion of backgroundand referencechecks. Proof of eligibility to work in the United States. Expected Work Hours and Location Generally, 8:00 am to 5:00 pm with occasional nights and weekends. Days and hours are subject to change with notice. May be considered for hybrid work after six (6) months of employment.
