Position Details

Shift: Day 5x8-Hour (08:00 - 16:30)

Start Date: ASAP

Floating Expectation: No

First Time Traveler: Yes, but first-time travelers must be screened

Weekend Requirement: Typically one full weekend every four weeks, subject to departmental needs

Qualifications

Years of Experience Required: 2 years of acute medical Care Manager experience performing Care Coordination and Discharge Planning
California State License: Required (Pending licenses accepted, but must be licensed upon start date)
Experience with Patient Ratios: Days: 1:25

Key Responsibilities

Acute Hospital Care Coordination and Discharge Planning
Assessing patients and determining medical necessity per evidence-based guidelines
Maintaining compliance with CMS, HIPAA guidelines, and department of health regulations
Implementing disease management strategies and care plans

Additional Information

Travelers may be expected to float between nearby facilities as required
Orientation includes 40 hours of training
Must commit to working three out of five designated holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
References required: Two references covering one year within the last three years
