Job DescriptionDavis Behavioral Health (DBH) is seeking a full-time Case Manager to join the FAST Assertive Community Treatment program. As part of a multidisciplinary team, the Case Manager is a bachelor's-level mental health professional who provides targeted case management, behavior management, skills development, and personal services. The case manager assists clients in effectively utilizing available community resources and may be provided in various settings. This position offers hope, guidance, and skills toward obtaining and maintaining independence in community living.Essential Job FunctionsThe FAST/AOT Case Manager must be able to complete the following essential job functions:Complete clinical documentation of all services within agency guidelines.Complete DLAs (or other agency-identified case management needs assessments) at the required frequency to create individualized goals and objectives.Coordinate with the client's treatment team and community agencies as needed to provide effective care.Be knowledgeable about the crisis system and know when to activate it.Participate in weekly staff meetings and regularly scheduled supervision.Other duties and responsibilities as needed and assigned.The FAST/AOT Case Manager is expected to uphold the core social work values of service, social justice, dignity and worth of the person, importance of human relationships, integrity, and competence. This includes maintaining client confidentiality, practicing within one's scope of competence, engaging in ongoing professional development, and advocating for the rights and needs of vulnerable individuals involved in the civil commitment process.Position-specific Job FunctionsIn addition to the essential job functions outlined above, the following responsibilities are assigned to your specific position.Understand and implement the Assertive Community Treatment (ACT) model of care in diverse community settings.Build and maintain therapeutic rapport with clients, respecting their self-determination while promoting engagement in treatment.Provide ongoing assessment of client wellness including physical, physiological and emotional needs and link clients with care for identified needs.Provide skill building support in individual and group settings to increase client independence.Conduct outreach and engagement with clients who are treatment resistant or who are experiencing an increase in symptoms or decrease in wellness.Attend weekly FAST team meeting Attend monthly 1:1 supervision as scheduled at orientation.Attend at least monthly staff meetings as scheduled.Demonstrate proficiency in use of company communication means including email, TEAMS and group text.Meet monthly and annual productivity standardsAbility to work with clients regardless of the clients age, sex or gender identity.Any other duties and responsibilities and needed and assigned.Introductory PeriodAll new employees at DBH have a 6-month introductory period to assess whether they can perform the essential functions of the job at the expected level, within the specified timeframes, and are a good fit for the job.Employment Classification & ScheduleThis position is classified as full-time, non-exempt, with a 40-hour per week schedule.Work schedules at DBH are established to meet the needs of our clients and the agency. Program Supervisors, with the approval of Program Directors, determine the specific schedules for their assigned departments and have the authority to ask employees to work a different shift and/or in a different location on a short-term or permanent basis to meet the departments' needs.The proposed schedule for this position is Monday through Friday during regular business hours. This position does require flexibility to meet client and program needs, including attending commitment hearings, staff meeting, and other tasks related to completing essential job functions.LocationThe office for the FAST/AOT Case Manager office is located on the DBH Layton Campus is located at 2250 North Robins Dr., Layton, UT 84041. Work locations include but are not limited to clients' homes, clients family member homes, hospital settings, courthouses, jails, DBH offices, and other locations to help the clients meet their needs.While this position will have office space within the agency, case management activities are expected to be performed across multiple settings including client homes and community locations. Significant fieldwork is required, including frequent travel throughout the county to conduct outreach, attend meetings, and provide direct client services. This position may require the use of a personal vehicle; mileage reimbursement is provided in accordance with organizational policies.CompensationThe hourly rate for this position is outlined below:BSW Degree + SSW License$25.50 per hour base wage + 1% per year of licensureNon-BSW Degree$21.50 per hour base wage + 1% per year of direct social work experience, up to 10 years.Internal Candidates Only: No Degree + TCM Certification$19.00 per hour + 1% per year of direct social work experience, up to 10 years.Required QualificationsEducation/Licensure/CertificationGraduation from a college or university with a bachelor's degree in social work (BSW).Preference given to those with current licensure in the state of Utah as an SSW.Candidates without an active SSW license may also be considered with the expectation that certification as a Behavioral Health Case Manager is completed within the first 30 days of employment (if not already obtained). Qualifications to become a Behavioral Health Case Manager through the Office of Substance Use and Mental Health (OSUMH) include:Completion of OSUMH's online training with a passing score on the exam400 hours of work, volunteer, and/or education experience in human services or a related field40-hour case management practicum, provided by DBHThe FAST/AOT Case Manager must complete and maintain certification in First Aid and CPR, Daily Living Assessment (DLA), and all mandatory training assigned to this position.Underfill OptionsPosition may be under-filled by an individual who has graduated from a college or university with a bachelor's degree in another social science field (i.e., psychology, criminal justice, sociology, etc.) AND has active licensure as a Social Service Worker (SSW) in good standing in the State of Utah or can meet criteria to obtain licensure within 12 months of hire.Internal Candidate Only Underfill OptionsCurrent DBH employees who are actively pursuing an undergraduate degree, have active Behavioral Health Case Management certification through the State of Utah in good standing, and meet all criteria for transfer eligibility as defined by DBH policy may be considered for case management vacancies at DBH.Professional ExperienceThe ideal candidate will have a working knowledge of mental health and substance use disorder services, community resources, experience working with clients that have severe mental illness, is hardworking, and can work independently as well as in a team environment. Experience in mental health case management or related behavioral health services preferred (1–2 years).Skills/Abilities/QualificationsThe Case Manager must possess strong verbal communication skills and demonstrate compassion, courtesy, and respect to all clients and their families. Exceptional interpersonal skills with the ability to establish immediate rapport with clients.Demonstrate the ability to work with SED/SPMI populations, substance use, and with individuals and families.Demonstrates the skills necessary to motivate and engage individuals.Knowledge of the techniques and principles of child and family development and family systemsKnowledge of trauma-informed care.Good organizational and practice management skills.Ability to work independently and effectively.Ability to establish and maintain effective working relationships with other staff and community/state agencies.Flexibility in days and hours available for scheduled work, including evenings.Spanish speaking is preferred.All employees at DBH must successfully pass a pre-employment criminal background check as well as a pre-employment drug screening. Since Davis Behavioral Health works with Federal grants the company and its employees must comply with the Federal drug laws, including no use of medical marijuana.Physical DemandsTasks require a variety of physical activities, not generally involving muscular strain, but do require frequent walking, standing, stooping, sitting, reaching, talking, hearing, and seeing. Common eye, hand, and finger dexterity are required for many tasks. Mental application utilizes memory for details, verbal instructions, emotional stability, and discriminating thinking. Periodic interaction with youth or adults in crisis requiring participation in de-escalation processes may occur causing stress or threat of physical injury.Must be able to lift up to 50 pounds and perform a range of physical tasks including walking, standing, driving, and navigating stairs.Position involves travel to varied community settings including hospitals, homes, courts, jails, and office spaces.Occasional high-stress situations may occur involving individuals in emotional or behavioral crisis.Must be comfortable working independently in the field with minimal direct supervision.BenefitsDavis Behavioral Health, Inc. offers an incredible valuable and comprehensive benefits packages to eligible employees.
