Today our agency continues to address the emotional and physical needs of children and families in Brooklyn, Queens and across Long Island through innovative treatments and life-changing interventions. We provide community-based prevention and family foster care services, group homes in the community for struggling children and families within the child welfare and juvenile justice systems, and short-term residential services for unaccompanied migrant children. Each year, MercyFirst serves more than 3,000 children, teenagers and families overcome enormous obstacles, re-imagine their futures and develop their full potential.
PROGRAM BACKGROUND:
MercyFirst’s NYC Residential Programs provide 24-hour residential care and supportive services for youth with a history of trauma and the presence of complex needs. Four agency-operated group homes provide youth with structure and supervision within family-like settings, designed to provide evidence-based treatment to promote emotional and physical safety and stability, practice positive coping skills, and build healthy relationships, as well as case management services to support discharge planning through family reunification or independent housing, and youth development services to foster growth and competency in daily life skills. The residential programs provide specialized residential foster care for teens who are entering the foster care system or require emergency replacement (YRC) and teens who are pregnant or parenting, and also accommodating their child (PPP).
POSITION SUMMARY:
The Director of Case Management will assume responsibility of the administration of all aspects of case management, educational and permanency planning for youth and families in the New York City Residential Programs. The qualified candidate will provide guided leadership and direct supervision for case planners and other services providers at one (1) Youth Reception Center and 3 Pregnant and Parenting (PPP) Residences. This individual will ensure that each program complies with all applicable local and state standards, instructions, procedures, regulations, and laws, as well as agency commitment to trauma-informed care. This candidate will also provide quality assurance reviews related to appropriate documentation, timely submissions and overall compliance with program’s performance measures. Director of Case Management will maintain collaborative and administrative contacts with ACS, OCFS, and other external stakeholders, providers and supports.
RESPONSIBILITIES:
- Ensures development of quality programming involving the required/recommended services for each child on assigned caseload (medical, developmental, clinical, and educational) and monitor the case progress and outcomes.
- Maintain compliance tracking and performance management systems to consistently monitor program outcomes and implement corrective action plans when necessary.
- Ensure assigned staff conduct Field Visits to family’s /client’s homes, schools, treatment centers, etc
- Facilitate the active participation of family members at Family Team Conferences (FTC) to build consensus around the development of Service Plans that address individual and family system needs and expectations to promote Safety, Stability, Permanency and Well-being outcomes.
- Training and Participation in the implementation of evidence-based/supported models of practice and treatment: Solution Based Casework (SBC); Attachment Regulation and Competency (ARC) .
- Responsible for review and monitoring of timely and accurate Progress Notes, Family Assessment and Service Plans, Court Reports and other required documentation. Adheres to Family First regulations related to QRTP and QRTP- exempt requirements.
- Participation in weekly rounds with program’s treatment team to ensure integrated approach to case progression.
- Responsible to maintaining case documentation and data entry into systems of record: Connections and ASARA
- Availability to assist with 24 hour on-call support to program personnel and clients
- Participation in weekly supervision, program meetings, trainings and workshops to enhance professional development and advance program development initiatives
- Establish and sustain a workplace environment that is respectful and supportive of staff, clients and families, community service providers, and other external stakeholders.
- Monitoring budgetary processes, including purchase approvals.
- Work in partnership with the VP of NYC Residential Services to develop strategies that promote growth and change within the program to respond to OCFS/DSS referrals
- Collaborate with agency-based and/or community-based service providers to advance family-specific needs and assist with obtaining services including, but not limited to safe housing, emergency financial assistance an, health care, mental health services, substance abuse services, education services, legal advocacy, and representation, parenting support, care management, prevention services,
- Hire, train, supervise and evaluate case management staff
- Participation in Agency-wide Management Team Meetings.
QUALIFICATIONS:
- Masters Degree in Social Work (MSW) or in a related Human Services field; NYS Licensure preferred.
- 3+ years of administrative/supervisory experience, specific to the field of child welfare.
- 5+ years of prior case management experience, specifically to New York City child welfare.
- Prior experience working with children and families known to the Child Welfare system, impacted by Abuse and Trauma Required
- Strong Engagement, Organizational and Communication skills.
- Knowledge of and commitment to child welfare best practice standards.
- Understanding and willingness to commit to the agency’s mission, core values, Sanctuary commitments, and Diversity, Equity, Inclusion and belonging (DEIB)
- Ability to accommodate a flexible work schedule, including evenings and weekends and to be on call for emergencies.
- Proficiency in Microsoft Office suite
- Demonstrate competency in child development.
- Experience in case work counseling, crisis intervention, safety planning, advocacy, concrete services and referrals, family support and psycho-education with youth and families in accordance with all applicable ACS, OCFS and Family Court standards, instructions and procedures.

SALARY RANGE:
$75,000 - $80,000 per year
Hiring Incentive - $1000 after 500 worked hours
