Requirements:

• 2 years of experience as a RN
• Qualified applicants MUST have at least 12 months of recent home health experience within the past 3 years
• Qualified applicants MUST have OASIS and HCHB experience
• Valid RN license in Indiana
• Be willing to obtain State licensure if not already licensed
