Registered Nurse In Home Health
Join our team as a Registered Nurse in Home Health, where you'll provide essential care to patients in their homes.
What You'll Be Doing
- Conduct home visits for patient assessments and care
- Manage a caseload of 5 to 8 patients daily
- Document patient progress and care plans using HCHB
- Collaborate with healthcare teams during periodic office meetings
- Travel up to 75 miles from the home clinic for patient visits
What We Need To See
- Active RN license required
- Current CPR and BLS certifications
- Minimum of 3 years of recent home health experience
- Proficiency in Homecare Homebase (HCHB)
- OASIS proficiency
