Preschool Services Coordinator (Case Management) for Riverside Preschool West

Summary Under the direct supervision of the Program Manager, this role coordinates the services and support available to consumers as outlined in the Lanterman Developmental Disabilities Services Act. The position provides no supervisory responsibilities and may perform related work as assigned. Location & Travel This position provides services to consumers in the Riverside West Area, which includes driving to Corona, Jurupa Valley, Mira Loma, Norco, and Perris. Mileage reimbursement for business travel is paid monthly. Hourly Range $26.31 - $40.82 Benefits & Sign‑On Bonus Sign‑on bonus: $250 after 90 days of employment with good performance. Sign‑on bonus: $500 after 6 months of employment with good performance. Essential Duties and Responsibilities Assume daily Consumer Services Coordinator responsibilities under the Program Manager’s direction. Evaluate consumer progress and plan as identified in Individual Program Plan (IPP) on an ongoing basis. Assure that consumers’ rights and dignity are maintained in the provision of services. Maintain contact with consumers and families as required. Provide advocacy services and attend all relevant meetings (i.e., IEP), prepare paperwork and notate the IPP as necessary. Seek out and effectively utilize generic resources, provide information and referral services. Conduct individual or family crisis intervention and document activities appropriately. Complete special incident reports on time and inform representatives of other programs/agencies as appropriate. Coordinate consumer assessment, including nursing, medical, psychological/developmental, education or vocational evaluations, and follow up as needed. Provide complete case coordination coverage when the primary Consumer Services Coordinator is absent. Coordinate, implement and update CDER, Annual Review and IPP processes. Serve Medicaid Waiver cases, completing required documentation timely and thoroughly. Complete placement and liaison activities as required. Act as liaison to at least two community agencies, parent groups or programs: attend meetings, provide program information and act as a resource. Provide written reports of activities to the Program Manager at least annually. Arrange placement of consumers in residential facilities or other programs as clinically indicated and approved by the Interdisciplinary Team. Provide case coordination services in all levels of community care facilities. Represent the regional center at case conferences and interagency meetings. Keep Program Manager informed of caseload status, community needs, liaison activities, and unusual or difficult cases. Act as program sponsor for new employees, providing training and completing required sponsor duties. Lead or participate in at least one Quality Assurance evaluation or audit of a community care facility, health care facility, or day program annually. Successfully complete all assignments arising from the agency’s Performance Contract. Complete administrative requirements, purchase of service forms, and consumer file documentation per agency policies and procedures. Reach at least 95 % of required case‑related paperwork within designated time frames. Coordinate the purchase of services identified in IPP, including documentation in quarterly reports. Prepare and present cases for compliance, eligibility, and other administrative reviews. Complete Title XIX Targeted Case Management documentation accurately and timely. Correct any out‑of‑date record portions to maintain complete records. Accept transfer to another unit (age group or catchment area) as required by the Inland Regional Center. Maintain interpersonal and community relationship skills daily. Comply with the Agency’s Injury and Illness Prevention Plan and report unsafe conditions. Serve as a team member, answering questions, sharing expertise, and contributing to team efficiency and training. Work cooperatively and effectively with others. Use office equipment appropriately, keep the work area neat, and observe all safety rules. Handle change well, remain flexible, and adapt to interruptions and new priorities. Keep the manager informed of pending work, progress, and encountered problems. Maintain good attendance and punctuality. Utilize agency IT systems as assigned, maintaining security and following protocols. Ensure program compliance with agency Personnel Policies and Procedures. Stay current and expand relevant knowledge, attending required training sessions. Organize travel efficiently and maintain a safe driving record. Use bilingual skills where applicable and required. Perform additional work as assigned. Minimum Position Requirements Master’s Degree in social work, psychology, public health nursing, teaching special education, early childhood development, or a related field from an accredited institution. Or BA/BS degree from an accredited institution plus one year of experience in case management, intellectual/developmental disabilities, social work, special education, early childhood development or related field. Bilingual preferred. Ability to handle change well and adapt to interruptions, new priorities, and new assignments. Ability to follow oral and written direction. Ability to establish and maintain effective working relationships. Ability to work cooperatively and effectively with others; answer questions, share expertise, and contribute to team harmony. Assure consumer rights and dignity are maintained in service provision. Good verbal and written communication skills. Full use of an automobile, possession of a valid California driver’s license and liability insurance as required by law; maintain a safe driving record. Case Management Trainee (CMT) For candidates who meet the education requirement for Preschool Services Coordinator but not the experience requirement, the department is accepting applications for a Year‑long Case Management Trainee position.
