Requirements Saint Alphonsus Medical Center in Ontario OR is seeking a locum OB/GYN physician to fill their need! Schedule:24 hour pager call for 5-7 day stretch minimum. Longer blocks increase your candidates' value with the client! ED GYN very low volume 1-2/monthSome clinic as needed to cover our 0.5 FTE provider when he is on Full ScopeLow volume with rare Call BackClinic Hours when needed - use Call Back rate for invoicing - Flexible but will most likely be 0830 - 1600Clinics are weekdays only when needed Dates Needed coverage needed Client wants to review providers that can cover at least 5-7 consecutive shifts per blockIdeal candidates can cover 10 consecutive shifts per block but client will review 5-7 consecutive will NOT be responsible for supervising other providersNICU Level 1Clinic Volume per day - Varies based on clinical needs but 15-18. Would increase if physicians want a higher volume of OB patients on the back end of the pregnanciesAnnual Deliveries the facility does - Approx. 360 deliveries around 30 per month and that number is droppingProvider will be expected to do deliveries during clinic hours - YesEMR: EPICFull Scope OB/GYN. The facility has a 10-bed LDRP Labor Delivery Recovery and Postpartum Unit with a 2-bed newborn nursery. Travel - 750/round trip cap to include all taxes and feesLodging - 179/night cap for hotels to include all taxes and fees. Preferred Lodging: Candlewood Suites Boise – Towne Square an IHG Hotel.Rental Car - To be reimbursed by facility not to be used with personal car/mileage reimbursement. 50/day cap including all taxes and fees regardless of car class. SUV/upgrades over cap will need to be preapproved will only be approved if needed for inclement weather.Mileage - IRS Standard Rate to be reimbursed by facility only to be used with personal vehicle not to be used with rental car. Bill Rates Needed for Rate must be included in the rates24 Hour On Call: 3900 2 gratis hours client's desired cap Holiday 24 Hour On Call Call Back: 350 client's desired capHoliday Call BackConversion fee 25k rate cap *Please Note: Trinity Health does not reimburse for any application or credentialing fees.
