Registered Nurse (RN) Case ManagerAscend Home Health and HospiceFull-TimePosition OverviewAscend Home Health and Hospice is seeking a compassionate and skilled Registered Nurse (RN) Case Manager to join our team and serve patients across Cache Valley and Box Elder Counties. In this role, you will provide high-quality, patient-centered care while coordinating services to support patients and their families throughout their healthcare journey. You'll play a key role in ensuring continuity of care, improving outcomes, and making a meaningful impact in the lives of those we serve.Key ResponsibilitiesProvide comprehensive nursing care to patients in their homes and/or assisted living settingsConduct initial and ongoing patient assessments and develop individualized care plansCoordinate care with physicians, therapists, social workers, and other team membersMonitor patient progress and adjust care plans as neededEducate patients and families on diagnoses, medications, and care plansComplete accurate and timely documentation in accordance with regulatory standardsParticipate in on-call rotation as neededQualificationsCurrent, active RN license in the state of Utah (or compact license if applicable)Strong clinical assessment and critical thinking skillsExcellent communication and organizational abilitiesAbility to work independently while collaborating with a teamValid driver's license and reliable transportationSchedule & BenefitsFull-time position, flexible scheduleCompetitive salary (based on experience)Comprehensive benefits package including:Medical, Dental, and Vision InsuranceGenerous PTO program401(k) with employer matchWhy Join Ascend?At Ascend Home Health & Hospice, our core values-Serve, Lift, and Unite-guide everything we do.
