Registered Nurse

Coordinates comprehensive case management and care for high-risk and at-risk patients, including assessment, individualized care planning, referrals, follow-up, and evaluation of patient outcomes.

Provides skilled direct nursing care across age groups, including triage, emergency treatment, clinical assessment, nursing interventions, and maintenance of accurate health records.

Coordinates multidisciplinary services and continuity of care by collaborating with patients, families, providers, specialty services, outside facilities, transportation resources, and other healthcare agencies.

Provides patient and family education, counseling, and preventive health support using culturally appropriate approaches to promote self-care, disease prevention, treatment adherence, and improved health outcomes.

Supports quality improvement, patient safety, and administrative/community outreach activities through documentation review, performance improvement efforts, staff and community education, and collaboration with the multidisciplinary healthcare team.
