Registered Nurse - Pediatrics

POSITION SUMMARY The Registered Nurse (RN) will play a pivotal role in guiding patients and their families through the healthcare system. The RN will be a vital member of the multidisciplinary healthcare team, dedicated to delivering high-quality nursing services across adult primary care, women's health, and pediatrics. The RN will conduct thorough assessments, perform essential clinical procedures, facilitate comprehensive care plans, and collaborate with interdisciplinary team members to ensure seamless care integration and improve health outcomes. OUR CORE VALUES Inclusion Nurture Service Purposeful Integrity Resilient Engaged QUALIFICATIONS Graduation from an accredited nursing program Licensed with the AZ State Board of Nursing and in good standing Two to five years of experience in a clinical setting Demonstrated application of Nursing Planning (Assessment, Diagnosis, Goals/Outcomes, Implementation, Evaluation) Understanding of clinical methods and record-keeping systems Ability to implement supervision principles and practices Proficiency with Microsoft Office (Outlook, Word, Excel) and the Patient Management System Analytic reporting skills integrating multiple data elements Familiarity with Adelante Healthcare mission, vision, and core values Maintenance of confidentiality, privacy, and security of protected information Excellent verbal and written communication skills Willingness to travel to all health centers as needed Valid Arizona driver's license and active automotive insurance Certification in CPR and AED per American Heart Association and Red Cross guidelines Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for specialty behavioral health locations Participation in Community Events as required POSITION RESPONSIBILITIES Patient Care Responsibilities Perform triage phone calls and walk-in patient assessments for adult primary care, women's health, and pediatric populations. Assist providers with procedures such as wound care, urinary catheterization, suture/staple removal, wart removal, lacerations, skin biopsy, abscess incision & drainage, toenail & skin tag removal, LEEP, colposcopy, and IUD insertion. Perform EKG, peak flow, spirometry, albuterol & oxygen treatments, and document results in the electronic health record. Clinical Collaboration Review and act on tasks/PTAs sent by providers. Assist with point-of-care testing (HbA1c, UA, HCGs, hemoglobin, fecal occult blood, lead, PKU, HIV, strep). Support vaccine and medication administration, including NG-G-tube (Rota vaccine). Perform PPD placement, flu and COVID testing. Facilitate the referral process per AHC standing orders and procedures. Develop Self-Management Goals (SMGs) in collaboration with patients using SMART criteria. Communicate lab and diagnostic results to patients as needed. Quality Improvement and Coordination Collaborate with the interdisciplinary team to improve the Uniform Data System (UDS) quality metrics. Improve care delivery integration and close HEDIS care gaps. Coordinate outreach to high-risk patients and manage transitional care with hospitals and skilled nursing facilities. Assist with insulin titration education and follow-ups for diabetic patients. Identify barriers to care and social determinants through PRAPARE survey data. Support high-risk prior authorizations, DME, and short-term disability needs. Ensure compliance with Joint Commission, HRSA, and AHC standards. Leadership and Accountability Provide leadership for service coordination, skill-mix definition, and delegation among licensed and unlicensed staff. Maintain accountability for all nursing services and associated patient outcomes under clinical provider direction. Patient Education and Reporting Provide education on STIs and public health reporting as needed. Build relationships with external health care organizations to secure additional resources for patient care. Administrative Responsibilities Obtain medical records, labs, and diagnostic results when needed. Adopt Adelante Healthcare mission, vision, and core value policies in all activities. Ensure proper and complete documentation using approved templates in the electronic health record. Conduct medication reviews, reconciliation, and refills. Evaluate clinical resources and support development of new tools, forms, and procedures. Participate in team meetings as required. Skills & Competencies Apply critical thinking and organizational skills to all nursing tasks. Enhance patient safety and quality by setting goals, implementing, and evaluating professional nursing services per board and organizational standards. Family Medicine (FM)/Internal Medicine (IM) RN Collaborate on Medicare Wellness Visits: conduct assessments, complete surveys, review charts, and document in the electronic health record. Provide direct nursing care to adult patients, including follow-up coordination, education, triage, assessments, medication administration, vaccinations, record management, and quality improvement. Women's Health RN Perform and interpret NSTs for pregnant patients. Assist with pap smears, breast exams, and contraceptive education. Provide direct nursing care and education for women's health patients. Pediatrics RN Provide direct nursing care to pediatric patients: coordination, education, triage, assessments, record management, medication administration, vaccinations, and quality improvement. Educate caregivers on child health, disease prevention, nutrition, and wellness practices. Assist with pediatric procedures such as circumcision. Quality Methodologies Maintain knowledge of PDSA, DMAIC, ADKAR, and SMART models. Collaborate in developing continuous quality improvement plans. NON-ESSENTIAL SKILLS & EXPERIENCE Electronic Health Record experience. Bi‑lingual (Spanish/English). Additional language proficiency besides English. Bachelor’s degree preferred. Other: advocacy experience, fluency in Spanish, job-related certifications. ADDITIONAL DUTIES & RESPONSIBILITIES Other duties as requested or assigned.
