$5,000 Hiring Bonus - FULL-TIME/YEAR-ROUND POSITION BASED OUT OF DODGE CITY, KANSAS WITH TRAVEL TO SURROUNDING COUNTIES
Up to $5,000 relocation assistance
Social Worker/Social Services Essential Duties and Responsibilities:
Responsible for the development and implementation of individualized programs, including case management for children enrolled in Infant Toddler Services in accordance with Arrowhead West, Inc.’s (AWI) corporate policies and procedures, KDHE infant-toddler regulations, and any applicable licensing and accreditation standards.
Serves on a multi-disciplinary team. Provides comprehensive and integrated support as outlined in the Part C Program Manual for Infants/ Toddlers in Kansas. Assists families in identifying available services needed and referral process, including coordination with medical and health providers.
Maintains accurate records, data logs, daily sheets, and Medicaid billings of children on assigned caseload in accordance with agency policy. Maintains case files and serves on case record review committee.
Serves as a Family Service Coordinator for assigned families requesting services.
Coordinates the development and implementation of the Individual Family Service Plan (IFSP) for children on caseload within the 45-calendar day time period from receipt of referral for the initial evaluation. Responsible for six-month reviews of the IFSP and coordinating annual evaluations.
Provides developmental supports for eligible children from birth to age three in the family’s natural environments and daily routines by using a primary coaching model.
Helps design the learning environment and activities that promote the child’s acquisition of skills in a variety of developmental areas. Provides families with information, appropriate referrals, skills and support related to enhancing the developmental skills and wellness of the child.
Participates in child-find activities with other community agencies as assigned and will refer identified children for an evaluation as soon as possible, but no more than seven days after the child was identified.
Conducts evaluations and ongoing assessment in all the areas of development for children enrolled in Infant Toddler Services.
Serves as AWI representative on the local interagency coordinating council (LICC) as assigned. AWI staff may not serve as the Council Chairperson.
Facilitates the development of a transition plan to Part B special education programs at least 90 calendar days before the child’s third birthday.
Ensures that follow-along visits are conducted for children dismissed or referred from Infant Toddler Services.
May serve as an interpreter for Infant Toddler Services and CDDO families.
Makes recommendations to the Infant Toddler Services Manager regarding problems, needs, or opportunities related to the provision of infant-toddler services.
Performs other functions as assigned by the Infant Toddler Services Manager.
Social Worker/Social Services Employment Standards:
General Requirements: The requirements listed below are representative of the knowledge, skill, and/or ability required. Individual must be able to effectively communicate in English, including the ability to read and write in English.
Education and Training: Masters in Social Work and experience in a special education setting or working with Infants and Toddlers. Ability to work well with other professionals. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of parents or employees of organization. Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Preference may be given to qualified individual who is bilingual in Spanish.
Certificates, Licenses, Registrations: Current Kansas license issued by the BSRB at the Masters level. Valid driver’s license (safe driving record, Department of Motor Vehicle check).
Physical Demands: While performing the duties of this job, the employee is regularly required to sit and talk and hear. The employee frequently is required to reach with hands and arms and stoop, kneel, crouch, or crawl. The employee is occasionally required to stand; walk; and use hands to finger, handle, or feel objects, tools, or controls. May need to climb up/down stairs. The employee must frequently lift and/or move up to 30 pounds and occasionally up to 40 pounds. Ability to drive car. Position requires travel within assigned area.
Work Environment: The noise level in the work environment is usually moderate. Services are provided in the child’s natural environment.
Salary Range: Determined by the local Special Education Cooperative’s salary schedule.
