Bilingual Complex Care Case Manager
Marin Community Clinic
Marin Community Clinics is seeking an Associate Case Manager to join our Complex Care team in Marin County. You will support a caseload of 30-40 patients, develop care plans with patients, address social factors, and coordinate care with a multidisciplinary team to improve health outcomes. The role requires MA/CHW/BS in social work or equivalent, 1–2 years’ relevant experience, Spanish language ability, and proficiency with Microsoft Office. #J-18808-Ljbffr Marin Community Clinics
