- Provide physical therapy services to patients according to the Home Health Plan of Care under the supervision of a licensed Physical Therapist
- Perform physical therapy procedures in the patient's home
- Administer non-complex therapeutic exercises, massage, traction, heat, light, cold, water, and electrical modalities to patients with relatively stable conditions
- Assist and motivate patients in improving functional mobility and independence
- Teach basic body mechanics to patients and families
- Instruct patients and family members in the proper use and care of prosthetic devices and therapeutic appliances
- Observe and document patient progress and response to treatment, reporting, findings and any condition changes to the supervising Physical Therapist
- Modify the Home Health Plan of Treatment as authorized by the physician and Physical Therapist
- Participate in home health care conferences and interdisciplinary team meetings
- Communicate plan of care updates to patients and families
- Prepare and submit clinical documentation and progress notes in a timely manner in accordance with Otterbein Home Health policies and state/federal regulations
- Complete a minimum of twelve (12) hours of in-services annually
- Education: Graduate of Physical Therapist Assistant program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE)
- Experience: Minimum of three (3) years full-time PTA experience OR At least 2,000 hours of patient-related care experience
- Licensure/Certification
- Current Ohio license as Physical Therapist Assistant (or application in process)
- Valid Ohio driver's license
- Reliable transportation with auto insurance
- Satisfactory criminal background check
- Current CPR certification
