We are currently accepting applications for a Registered Nurse (RN) MDS Assistant within our long-term care and rehabilitation facility specializing in geriatric care.

We are seeking a dependable, organized, and detail-oriented nurse to assist with the MDS process, resident assessments, care planning, and documentation. The ideal candidate will work closely with the MDS Coordinator and interdisciplinary team to help ensure assessments are completed accurately and on time while supporting quality resident care and regulatory compliance.

Qualifications

• Must hold a current, active Alabama Registered Nurse (RN) license
• Previous long-term care experience preferred
• Previous MDS experience preferred
• MDS transmission experience preferred
• Knowledge of MDS 3.0 and PDPM preferred
• Strong organizational, communication, and documentation skills
• Ability to manage multiple deadlines and work effectively with an interdisciplinary team

Essential Job Functions

• Assist the MDS Coordinator with completing and coordinating resident MDS assessments
• Help maintain an accurate assessment schedule, including MDS and care plan due dates and deadlines
• Review resident documentation to help identify changes in condition that may require assessment or care plan updates
• Monitor quality assurance documentation, including weight records, incident reports, shift reports, skin condition reports, and other clinical documentation
• Assist with care plan development and updates
• Participate in PPS, rehabilitation, and interdisciplinary team meetings as needed
• Work closely with nursing, therapy, dietary, social services, and other departments to ensure resident assessments accurately reflect current needs
• Assist with maintaining compliance with applicable state, federal, and CMS requirements
• Support accurate documentation related to PDPM and reimbursement
• Perform additional MDS-related duties as directed by the MDS Coordinator or Director of Nursing
