The cover page needs to be 100% accurate with all requests noted. · Candidates must be able to start on the listed start date, or no more than one week later · RTO must be on the cover page and cannot exceed 7 days. No RTO during the first week of assignment · Please list all facilities the candidate is willing to work in.
