Lead Addiction Recovery Case Manager II

Job Title: Case Manager II Position Wheeler Mission Ministries is a privately funded 501(c)(3) non‑profit, evangelical Christian ministry whose designated purpose is religious. We are a Christ‑centered ministry dedicated to sharing the Gospel and helping the homeless and impoverished. Exempt/Non‑Exempt: Non‑Exempt Reports to: Program Manager Full or Part‑time: Full Time Team: Center for Women and Children Position Summary Is responsible for providing a continuum of care for guests of Wheeler Mission through case management, including a thorough assessment and identification of each guest's resources, talents, skills and interests, and developing a plan to identify and achieve personal goals. Key duties include general case management activities, mentoring, discipleship, counseling, and teaching and managing groups when needed. Key Duties and Responsibilities Case Management for Guests Assessment and identification of each guest’s resources, talents, skills, and interests. Develop a care plan with written goals and methods to help the guest achieve those goals. Monitor the effectiveness, appropriateness, and efficiency of the plan, modifying as needed. Advise guests on best practices for success and achievable goals. Identify resources and needs, lead crisis intervention, provide information, and referrals. Program Oversight Monitoring guest progress. Program compliance. Facilitating small groups. Assisting guests in finding a foundation in Christ. Coordination of Services Working collaboratively with other program staff members. Collaborating with other agencies to coordinate services designed to create a network. Advocating for guests when necessary. Administrative and Other Record keeping for guests through maintaining appropriate documentation, entering information in database that documents progress. Legal responsibilities, including taking appropriate action according to the established protocol and maintaining guest confidentiality in all areas following biblical definitions of ethical conduct and behavior, and ensuring adherence to all facility policies. Participate in multidisciplinary guest case conferences that are designed to share information beneficial to the guest. Self‑care includes taking time to prepare for each day with study and prayer, serving in unity with other staff members and practicing healthy work/life balance, including participation in a faith community. In addition to the above key responsibilities, the Case Manager II will: Lead primary program elements. Take the lead in resource development with outside agencies and resources. Attend court hearings. Lead in gathering/maintaining program stats/outcomes. Mentor Case Manager I's. Reporting Relationships The Case Manager may be responsible for providing task supervision of program assistants, interns, and volunteers. Competencies Adaptability Organizational agility Listening Guest/Customer focus Collaboration Patience Relates well to others Prioritizing Conflict resolution Compassion Networking Education/Experience Minimum qualifications: 2 years of social service/ministry experience, specifically counseling, mentoring and discipling. 1 year of experience working with people who may have experienced substance abuse, mental health concerns, trauma, domestic violence, or homelessness. Bachelor’s degree for Level II (preferred). Required Knowledge Areas for Success Knowledgeable about community housing and employment resources and help guests connect to housing and employment. Demonstrated ability to assist guests in obtaining appropriate documentation, utilizing resources, and job searching effectively. Best practices in case management and faith‑based concepts to serve guests with excellence. De‑escalation, trauma‑informed care, mental health support. Technical / Software Knowledge Intermediate proficiency in Microsoft Office Suite (email, Word, Excel). Intermediate proficiency in Resident Database and basic ability in Paycom. Grace and truth. Physical Requirements Walking short distances with the ability to ascend and descend ten flights of stairs. Physical dexterity sufficient to type and operate standard office equipment. Physical ability to perform normal office functions with endurance to remain standing for long periods of time. Strength to lift office supplies and equipment up to 20 pounds. Ability to drive a vehicle with multiple occupants in a city environment. Other Requirements for Employment This is a full‑time 40‑hour per week position, typically staffed throughout the week with some evening and weekend hours required. Must possess a valid state‑issued driver’s license and have a driving record without major moving violations, including convictions for DUI, reckless driving, or driving while suspended. Must be insurable according to standards established by Wheeler Mission’s risk management insurance agency. Conflict of interest policy. Confidentiality policy. Post‑offer acceptable background check required for all positions. All employees must sign and adhere to WM Drugs and Alcohol policy and remain free from addiction to drugs or alcohol and illegal drug use.
