Position: Home Health Physical Therapist Assistant (PTA)

Location:

Service Areas: Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek, Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, Bay Point - California

Position Details:

We are seeking a dedicated Home Health Physical Therapist Assistant to join our team. This position involves delivering exceptional patient care in a home health setting.

Schedule:

• Shift: Day, 5 days a week (8-hour shifts from 08:00 AM to 04:30 PM)
• Guaranteed Hours: 36 hours per week
• No on-call requirements
• Weekends: Opportunity to pick up additional patients on weekends to meet productivity requirements

Responsibilities:

• Provide one-on-one care to patients in a home setting.
• Visit 5-6 patients each day, focusing on achieving 27 productivity points per standard.
• Maintain timely communication and accurate documentation of patient care.
• Utilize provided documentation tools and return them upon completion of assignments.

Qualifications:

• Minimum of 1 year of experience in Home Health.
• Valid California Physical Therapist Assistant License.
• BLS certification is required.
• Reliable transportation is essential for patient visits.
• Experience with various patient diagnoses, including cognitive disorders and post-operative care.

Additional Information:

• Staffing system effectively groups clients to minimize driving time.
• Maximum driving radius may be up to 50+ miles on occasion.
• Patients of varying ages and diagnoses will be cared for, including those with conditions such as Parkinson's disease, spinal cord injuries, and more.

Any color scrubs are acceptable.
