About the Role: As a Registered Nurse at Lake Pointe Rehabilitation & Nursing Center, you will be responsible for providing direct patient care, developing and implementing individualized care plans, and collaborating with our interdisciplinary team to ensure the best possible outcomes for our patients. You will have the opportunity to work closely with our team of physicians, therapists, and caregivers to address the complex needs of our patients and promote their independence and well-being. Responsibilities: Provide direct patient care to a diverse population of short-term rehabilitation and therapy patients Develop and implement individualized care plans that meet the unique needs of each patient Collaborate with the interdisciplinary team to ensure seamless care coordination and communication Conduct patient assessments, identify care gaps, and develop interventions to address these gaps Educate patients and their families on disease management, medication regimens, and self-care techniques Maintain accurate and up-to-date patient records, including progress notes, treatment plans, and discharge summaries Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction Communicate effectively with patients, families, and healthcare team members to ensure exceptional patient care What We Offer: Salary: $36 - $38 hourly.
