About the Position

AlliedTravelCareers is working with United Health Care Staffing to find a qualified Physical Therapist (PT) in Medford, Oregon, 97502!

• Deliver skilled physical therapy services in patients’ homes
• Evaluate and treat patients with orthopedic and neurologic conditions
• Develop and implement individualized care plans
• Perform patient assessments and track progress
• Educate patients and caregivers on home exercise programs
• Complete accurate and timely documentation (OASIS required)

Requirements

• Minimum 1 year of Home Health PT experience (Required)
• BLS Certification
• OASIS Documentation Experience (Required)
• Valid state license (OR or pending accepted)
• Must have reliable transportation
