Social Worker MSW

Job Summary The Social Worker (MSW) is responsible for non‑clinical biopsychosocial assessments and interventions on inpatient floors and the outpatient emergency department. Responsibilities include crisis intervention, patient/family interventions, high‑risk screening, emotional support, and referrals for financial resources. The MSW assists with post‑acute care placement, works in intensive care units, trauma, oncology, NICU, transplant, and ED, and responds to physician and discharge‑planning consults throughout the facility. Responsibilities Collaborate with RN case managers, acute care nurses, physicians, insurance companies, and other staff to proactively identify and address patient needs. Identify and address barriers to discharge and recovery including financial, emotional, psychosocial, medical, and spiritual barriers. Maintain knowledge of Medicare, Medicaid, private payer regulations, and managed‑care concepts related to length‑of‑stay management. Perform thorough assessments and develop discharge plans for high‑risk patients with psychosocial needs. Coordinate and facilitate meetings between the care team and patients/families/caregivers; assist in decision making honoring preferences and values. Provide psychosocial interventions, education, counseling, advocacy, and referrals for emotional, social, and financial aspects of care. Offer emotional support during complex or emotionally charged issues such as end‑of‑life, terminal illness, death notification, and crisis interventions. Qualifications Basic Cardiac Life Support certification within 7 days of employment start. LMSW/RCSW‑I, Licensed Master Social Worker or Registered Clinical SW‑Intern. Master’s degree in Social Work.
