Hospital-based trauma call coverage

Licensure Requirements

Active Arizona Medical License
- OR
IMLC Letter of Qualification (LOQ) in Hand
Facility is willing to license qualified providers

DEA Requirements

DEA Required

Board Certification

Board Certified or Board Eligible in Plastic Surgery - Required

Experience Requirements

Minimum 1 Year of Plastic Surgery Experience
Level I Trauma Experience Required
Must be comfortable managing:
- Facial Trauma
- Hand Trauma
- General Trauma Reconstruction
