LPN / Licensed Practical Nurse - Home Health

We are seeking a dedicated Licensed Practical Nurse (LPN) for a full-time Home Health role in Charlottesville, VA. Responsibilities Providing 1:1 patient care following individualized care plans while maintaining dignity and independence Administering medications, feedings, oxygen, and ostomy care, and assessing patient outcomes Documenting all nursing actions, communicating with family, case managers, and physicians, and updating on patient progress and condition changes Coordinating supplies, medications, and ensuring patient needs are met per physician instructions Qualifications Licensed Practical Nurse or equivalent with current registration and license in the state Valid driver's license, acceptable driving record, and proof of insurance CPR certification Preferred: 1+ years of nursing and home health experience Key skills and attributes patient-centered care, documentation accuracy, effective communication, ability to work independently, and team support. #J-18808-Ljbffr Alpha Home Health and Hospice
