Job Title: Registered Nurse Case Manager Profession: Case Manager RN Specialty: Nursing Duration: 13 weeks Shift: Monday - Friday Hours per Shift: 8 hours Experience: Minimum of one year as a clinical nurse in a hospital required. Nine months of experience in home care preferred. License: Current RN License in New York required. Certifications: BLS certification required. OASIS experience required. Telemetry experience required. Must-Have: Certified Home Health Care experience required. EPIC or Netsmart experience preferred. Strong interviewing, clinical assessment, and communication skills required. Ability to exercise initiative and independent judgment required. Ability to build and maintain professional relationships required. Current driver’s license required. Ability to work independently with minimal supervision and manage a variety of assignments required. Description: Develop and revise plans of treatment and discharge in collaboration with patients, families, physicians, care delivery teams, and third-party payors. Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with defined timeframes. Process orders and notify physicians of patient needs and changes in condition. Provide a respectful environment for patients and protect their privacy and confidentiality. Initiate appropriate preventive and rehabilitative strategies. Provide specialized nursing services as required. Consult and educate patients, families, and team members about disease processes, self-care techniques, and preventive strategies. Supervise Licensed Practical Nurses to ensure patient health, safety, and adherence to the care plan. Direct, coordinate, evaluate, and supervise the quality of patient care services provided. Interact with patients, physicians, referral sources, and others to maintain positive relationships. Support collaborative and customer-focused planning and case management to meet individual health and service needs. Ensure compliance with state and federal legislation and complete all necessary clinical records promptly. Maintain current knowledge of available community-based services. Demonstrate flexibility in visiting patients based on their needs. Resolve problems at the point of contact. Ensure appropriate authorizations and releases of information are obtained. Attend patient care conferences for assigned cases. Monitor quality indicators and customer satisfaction related to the RN function. Understand the RN function's impact on patient care, readmission, productivity, utilization, revenue, and expenses. Participate in efforts to reduce avoidable costs and errors. Respond to findings from quality management reviews and ensure resolution of potential issues. Participate in quality assessment and performance improvement initiatives. Maintain productivity requirements. Ensure visit documentation reflects the medical necessity for home care services. Promote an environment of professional growth, learning, trust, and mutual respect among employees. Participate in education and training programs as needed. Demonstrate commitment to professional growth and competency. Be responsible for the Key Performance Indicators established by management. Complete other duties as requested.
