Role Overview The Registered Nurse provides and directs nursing care to patients in their homes, ensuring compliance with laws, regulations, and agency policies. Key Responsibilities Deliver high-quality clinical services within scope of practice and infection control standards Coordinate care from admission to discharge with the care team Perform clinical assessments per program and payer requirements Verify patient eligibility and medical necessity for services Create and revise care plans in collaboration with community providers Adjust care plans based on patient needs with physician approval Provide care in diverse home situations, respecting all clients regardless of background Qualifications & Skills Current, unrestricted RN licensure in the practice state Valid driver’s license, vehicle insurance, and access to reliable transportation Preferred: 1+ years RN experience, CPR certification, strong communication, organizational skills, and ability to work independently Join a supportive environment that recognizes performance, offers development opportunities, and provides comprehensive benefits.
