- Conduct thorough evaluations of patients in both inpatient and outpatient environments, customizing treatment plans for individual needs
- Administer physical therapy interventions tasked with enhancing mobility, strength, balance, and overall function
- Work hand-in-hand with nurses, physicians, case managers, and a variety of rehabilitation experts to optimize patient outcomes
- Inform and educate patients and their families regarding therapeutic goals, discharge processes, and essential home exercises
- Maintain diligent documentation of evaluations, treatments, and patient progress, adhering to facility and regulatory guidelines
- Participate in team rounds and interdisciplinary meetings to promote streamlined and coordinated care
- Doctorate in Physical Therapy (DPT) or an equivalent degree recognized by accredited institutions
- State license to practice as a Physical Therapist
- CPR certification preferred
- Additional specialized certifications (neurological, geriatric, etc.) are advantageous but not mandatory
- Previous experience is strongly preferred
- Aptitude for treating patients with complex medical and mobility challenges
- Excellent communication and collaboration skills
- Proficiency with digital charting and clinical documentation systems
