What You’ll Do
Perform comprehensive physical therapy evaluations and develop individualized plans of care
Evaluate and treat patients to improve mobility, strength, balance, endurance, and functional independence
Develop evidence-based treatment plans and modify them based on patient progress
Educate patients and caregivers on mobility, fall prevention, home exercise programs, and safe functional movement
Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team
Complete accurate and timely documentation while maintaining Medicare and agency standards
Qualifications
Current Florida Physical Therapist (PT) license
Valid driver’s license and reliable transportation
Excellent communication and organizational skills
Home health experience preferred, but not required
Experience with WellSky or another home health EMR is a plus
