Nurse Practitioner or Physician Assistant

Humana's Primary Care Organization is seeking a full‑time Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to join our clinic in beautiful and sunny Jacksonville, FL. Responsibilities Provide comprehensive care to seniors following clinical standards. Collaborate daily with the care team to ensure coordinated, high‑quality care. Manage referrals, hospital/SNF coordination, DME, and home health services. Document care with support from quality‑based coders. Participate in on‑call rotation and support growth plans at your center. Apply experience in value‑based care. Work on site. Qualifications Master’s degree in nursing with board certification. Active, unrestricted NP or PA license in the state of Florida. Experience in value‑based care, primary care or Medicare Advantage settings. Active DEA license and Medicare/Medicaid provider numbers, or ability to obtain them. Familiarity with HEDIS quality indicators and Medicare guidelines. Pay & Incentives Pay Range: $123,800 - $170,300 per year (full time, 40 hours per week). This role is eligible for a bonus incentive plan based on company and/or individual performance.
