In this role, you will provide hands-on personal care and emotional support to patients facing life-limiting illnesses, helping them remain comfortable and maintain their dignity in the place they call home.If you are passionate about making a meaningful difference in the lives of patients and their families, we'd love to hear from you.## Responsibilities* Provide personal care, including bathing, dressing, grooming, oral hygiene, toileting, and skin care.* Assist patients with mobility, transfers, repositioning, and range of motion exercises.* Observe and report changes in patient condition to the hospice Registered Nurse.* Document all patient care accurately and in a timely manner.* Offer compassionate companionship and emotional support to patients and their families.* Assist with light housekeeping tasks that contribute to patient comfort and safety.* Follow each patient's individualized plan of care.* Maintain patient confidentiality and comply with HIPAA regulations.* Collaborate with the interdisciplinary hospice team to provide exceptional patient care.* Follow all agency policies, infection prevention practices, and safety standards.## Qualifications* Current Illinois Certified Nursing Assistant (CNA) certification.* Current CPR certification.* Valid driver's license, reliable transportation, and current auto insurance.* Ability to travel throughout the assigned service area.* Strong communication and interpersonal skills.* Compassionate, dependable, and able to work independently.* Hospice or home health experience preferred## Benefits* Competitive pay* Mileage reimbursement* Medical, Dental, and Vision Insurance* Ongoing training and professional development* Supportive team environment* Opportunity to make a meaningful impact every day## Why Care2You?At Care2You, we are committed to providing compassionate, patient-centered care while creating a supportive workplace where our team members can thrive.
