HIV Support Services Case Manager - Access & Care

Prism Health North Texas, Inc.

Prism Health North Texas, Inc. seeks a Non-medical Case Manager (N-MCM) to coordinate psychosocial support for people living with HIV/AIDS. You will provide client-centered case management and guidance across medical, social, community, legal, financial, employment and vocational services to improve access and retention in care. The role covers agency, offsite, partner location work and home visits, with ongoing needs assessment, care-plan monitoring and referral outcome documentation aimed at #J-18808-Ljbffr Prism Health North Texas, Inc.
