Virginia Licensed Practical Nurse

To support the LOCERI/LTSS Waiver program, the full-time Virginia Licensed Practical Nurse will coordinate and implement the EPSDT Member Outreach Program, ensuring compliance with health screenings and vaccinations while working remotely. Key responsibilities Facilitate and coordinate the Early and Periodic Screening, Diagnostic and Treatment (EPSDT) Member Outreach Program Manage data entry and monitoring of reports to enhance utilization of EPSDT services and maintain tracking systems for compliance Provide complex case management for members with special healthcare needs and conduct community engagement to promote EPSDT awareness Required qualifications Licensed Practical Nurse (LPN) license in Virginia or Compact/Multistate preferred Associate's degree in Nursing preferred; Bachelor's degree in a related field or equivalent experience accepted Minimum of 3 years of Managed Care experience required Experience in Case Management and Long-Term Services and Supports (LTSS) waiver preferred Strong understanding of EPSDT and childhood health initiatives preferred
