Physical Therapist (PT) - Must Be Licensed

A great Skilled Nursing Facility (SNF) / Long Term Care (LTC) is looking for an experienced or newly graduated Physical Therapist (PT) to join their growing team. The Physical Therapist is responsible for assessment of referred patients requiring rehabilitation services, providing direct patient care to assess medical condition, functional capabilities, limitations, and potential for rehabilitation. The PT will establish and administer a treatment program, set realistic goals, document, and educate patients and families. This is a direct hire/permanent opportunity. Responsibilities Assess referred patients to determine medical condition and functional capabilities. Establish and administer treatment programs with goals tailored to the patient's capacity and tolerance under physician direction. Set realistic and achievable goals, document and record patient condition. Educate patients and families in appropriate physical therapy methods. Develop a plan of care for each patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs; plan, evaluate, and modify care to meet goals of interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient. Qualifications Graduated from an accredited school. Current state license as a Registered Physical Therapist or in the process of obtaining licensure. Current CPR certification. #J-18808-Ljbffr
