Job Title LPN Reports to Assisted Living Manager or Memory Care Manager

Position Scope The nurse collaborates with other healthcare professionals to treat patients with a range of injuries, illnesses, and disabilities. Their duties include communicating with doctors and other Registered Nurses about patient needs, administering IVs and medications to support patients’ health, and using medical equipment to monitor vital signs. RESPONSIBILITIES Supports the mission, core values and goals of the organization, upholding and promoting company culture and vision. Completes all hours of state-mandated continuing education each year. Promotes and displays a spirit of teamwork in the performance of daily duties through cooperative interactions with co-workers and other departments. Promotes the property in a positive manner and effectively communicates the organization’s values to residents, visitors, co-workers, and the community. Assist in the direction, coordination and evaluation of nursing care services provided to the residents. Perform physical examinations and diagnostic tests. Collect patient health history. Counsel and educate patients and their families on treatment plans. Implementation of nursing services, objections, and standards; resolving resident care issues. Complete resident charting as required. Administer medication, change wound dressings and care for the other treatment options. Read and interpret patient information, making treatment decisions when appropriate. Consult with supervisors and doctors to decide on the best treatment plan for the patient. Research ways to improve healthcare processes and improve patient outcomes. Performs other duties consistent with the position as assigned. QUALIFICATIONS Licensed as a Practical Nurse and in good standing with the Board of Nursing. Two (2) years’ experience in assisted living preferred. Must have excellent computer skills. Excellent written and verbal communication skills. Able to follow all established Federal, State and Local rules, regulations, and guidelines. Must have a strong understanding of the English language, sufficient to speak, read and write. PHYSICAL QUALIFICATIONS Walk/Stand/Sitting – must be able to continuously perform duties that incorporate walking, standing, or sitting. Environment Condition – must be able to perform work both inside and outside. Lift - constantly 0 – 10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds. Carry - constantly 0 – 10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds. Push - constantly 0 – 10 pounds, frequently 11 – 50 pounds, occasionally 51 – 100 pounds. Climb – must be able to climb stairs on a frequent basis, with the opportunity to utilize the elevator as an alternate resource if available. Bend – must be able to bend at the waist, knees, hips, and spine on a frequent basis; may require frequent manipulation of weights of 25 pounds or more. Twist or rotate – must be able to twist or rotate occasionally at the trunk, hips, knees, and neck, manipulating weights of 25 pounds or more. Reach: may occasionally reach overhead, requiring manipulation of weight of 10 pounds or less, and below the waist, requiring manipulation of weight of 25 pounds or more. Sensory Vision – must be able to read clearly with or without corrective lenses. Hear - Must be able to hear telephone, audible alarms, bells, and signals related to resident safety with or without hearing devices.
