Job OpportunityPosition Overview Seeking a compassionate Nurse Practitioner or Physician Assistant to join a busy, well-established Family Medicine primary care practice. This role provides comprehensive care to patients across the lifespan, from pediatrics through geriatrics, in a collaborative and supportive outpatient setting.Key Responsibilities & Practice HighlightsPerform comprehensive medical evaluations, diagnose conditions, and develop individualized treatment plans for patients of all agesManage chronic diseases including diabetes, hypertension, and asthma through ongoing care and medication managementCollaborate closely with physicians, nursing staff, specialists, and other care team members to deliver coordinated, patient-centered careAccurately document patient encounters, histories, and outcomes using EPIC EMREducate patients and families on diagnoses, treatment options, preventive care, and self-management strategiesStay current with evidence-based guidelines and clinical best practicesMaintain a patient-focused approach emphasizing communication, empathy, and quality outcomesAverage patient volume: ~18 patients per daySchedule & Work-Life Balance4-day work week Monday, Tuesday, Thursday, Friday 8:00 AM – 4:30 PMCandidate RequirementsCompletion of an accredited Nurse Practitioner or Physician Assistant programActive and unrestricted Indiana license (NP or PA)Valid DEA, RN/APRN license (for NP) or PA licenseStrong clinical judgment with ability to manage both acute and chronic conditionsExcellent communication and interpersonal skillsEPIC EMR experience preferred
